State orders transfer of patients from troubled Warren nursing home
The Ohio Department of Health has ordered that all sixteen Medicare and Medicaid patients be removed from a Warren nursing home.

WARREN, Ohio - Under orders from the state, all 16 residents are being removed from a Warren nursing home.
The action follows a recent state inspection of Cedarcreek Health and Rehabilitation on Tod Avenue.
The Ohio Department of Medicaid has sent a letter to each resident of Cedarcreek informing them that Cedarcreek Health and Rehab no longer meets requirements to be certified for Medicare or Medicaid payment.
The letter advises the residents that they have one week, until Feb. 19, to make other living arrangements.
The Area Agency on Aging has a transition team that will be working with the residents and their families during the transition.
"One of the steps we took was we contacted all of Trumbull County to get the number of beds open in the area so that we were prepared, and the residents absolutely can make the decision of where they want to go," said Theresa Knapik, Long Term Care Ombudsman Specialist.
On the most recent inspection of the facility investigators found a patient had not been treated for bed sores and another had severe weight loss and had developed bed sores. Knapik said this was strike three for Cedarcreek.
"They had tried to come back into compliance on two separate occasions, but they remained in the immediate jeopardy phase," Knapik said.
Cedarcreek is owned by New Beginnings, and one of it's other facilities, Campus Health Care in Liberty township was closed last month after New Beginnings filed for bankruptcy. Knapik says the future of Cedarcreek appears doubtful.
"Our understanding is that New Beginnings is allowing them to leave and is not pursuing any appeal rights," said Knapik.
