A court hearing has been scheduled on Wednesday for a Beloit man who is facing multiple charges in connection with a fatal crash that occurred in Carroll County. The crash resulted in the death of three people, including an East Liverpool man.

According to records from Carroll County Municipal Court, Douglas Mackey, a 54-year-old resident of Beloit, is facing charges of three counts of vehicular manslaughter, traveling left of center, driving an unsafe vehicle, and a seatbelt violation.

The charges stem from a head-on collision that took place on April 10 in Monroe Township along Route 542. The crash report states that Mackey was driving a tree service dump truck pulling a chipper/shredder, which veered into the opposite traffic lane, colliding with a Honda Odyssey minivan. Both vehicles sustained extensive damage.

The driver of the Odyssey, Randy Simmons, a 60-year-old resident of East Liverpool, was killed in the crash. Two passengers, identified as 18-year-old Tina Shelter and 16-year-old Kenneth Shelter from Hammondsville, Ohio, also lost their lives.

The accident report reveals that the tires on the truck were worn down.
