HUBBARD, Ohio -  A Hubbard woman is facing several charges after forging multiple doctor's excuses for her child. 

Angela Frazzini, 34, is facing charges of identity fraud, tampering with records, contributing to the delinquency of a child, and criminal simulation. 

Police were contacted on Sept. 27 by the Hubbard Diagnostic and Speciality Center after they spoke with the school, trying to verify a doctor's excuse. 

It was determined that Frazzini signed the excuse using a doctor's name at the center and had filled out the wrong dates on the excuse. 

The doctor in this incident did not want to press charges against Frazzini, instead of giving her a warning that future issues would result in charges.

Frazzini told police that she did write the excuse but did not think it was a big deal because her child did go to the doctor's office that day to pick up x-rays.

The school told police that forging excuses had been an issue in the past with Frazzini.

During further investigation, it was discovered Frazzini had forged another five excuses from the same doctor in Hubbard and an excuse from Akron Children's Hospital in the past year.

Police called Frazzini to meet on Nov. 14 to discuss the case, but she never showed up. 

A warrant was then issued for her arrest, and Frazzini was picked up on Tuesday. 

Frazzini appeared in court Wednesday and is due in court again on January 2.

Her bond was set at $10,000.
