Police officers in Youngstown are currently searching for a missing teen with a list of disabilities who escaped from a residential facility.

Devorah McFadden (16) was last seen on Monday Nov. 8 at Safehouse Residential Services on Eastview Dr. in Youngstown. She was placed there by the Licking County Child Protective Services Agency.

McFadden's mother tells 21 News McFadden has a history of running away. She tells 21 News that McFadden was given an ankle bracelet to monitor her location, but chewed it off before she escaped.

McFadden is 5'7" and 145 pounds. She has brown eyes and brown hair, but was last seen in a long black wig.  She was last seen wearing a jean jacket, jean shorts and black leggings.

McFadden's mother was in Youngstown Thursday and Friday putting up fliers throughout the area and distributing them to residents. She tells 21 News she is currently in Cleveland and will be traveling to various locations searching for her daughter.

Safehouse staff told McFadden's mother that they were "following guidelines from the Ohio Department of Jobs and Family Services" when asked about how they were handling the search.

Anyone with any information on McFadden's whereabouts should call 911 or 914-484-0147. There is a cash reward for information leading to her being located.
