Mahoning Trumbull Community Health is currently conducting a Community Health Assessment (CHA) and needs input from residents across the Mahoning Valley.

The CHA is performed every three years and helps identify the most significant health concerns for adults and families who reside in Mahoning and Trumbull Counties.

Mahoning Trumbull Community Health Partners want to hear from residents 18 years of age and older.

The information collected from this survey will help create and guide action plans to address the identified major community health issues in Mahoning and Trumbull Counties.

This process encourages connecting, coordination, and working together toward common goals to help strengthen the programs, services, and resources that create a healthy community.
